  • Please note the date that a vaccination clinic will be held at your child’s school. (The second dose will be administered at the West Allis Health Department.)

  • If you are interested in having your child vaccinated, please fill out the attached consent form and return it to the Main Office by the due date listed.

Please read the Emergency Use Authorization (EUA) attached which contains detailed information about the Pfizer vaccine and the HIPAA Privacy information below.  If you have further questions you may email COVIDvaccine@westalliswi.gov or call the Health Department and ask to speak to a Public Health Nurse at 414-302-8600.
