FORMS
The following forms are used at Nathan Hale High School. Please download the form as needed, fill it in, and bring the completed form (in person) to the main office. Main office hours are from 7:00 a.m. until 3:30 p.m. daily. Summer hours are from 7:30 a.m. until 3:00 p.m.
- Complete Enrollment Packet - These forms are used on Forms & Fees Day. For those students who enroll after the August registration day and are living in the West Allis-West Milwaukee School District, you may use these forms. Please understand that when you bring the forms to the main office, you will be asked to verify your residency. For verification purposes, we like to accept the most current WE Energies bill, which shows the owner's name and address of the residence.

- Emergency Contact Information Form - This double-sided form provides the school with the emergency contact information for each student. For parents, extended family, and friends that are listed on the form, it provides us with the contact information in case of some type of emergency that may happen at school. If contact information changes during the school year, please complete a new form at bring it to the main office.

- Asthma Care Plan - For those students who suffer from asthma, parents should complete this form so that the school knows what needs to be done in case of an asthma attack at school.

- Computer, Internet, E-Mail Acceptable User Policy Sign-Off Form - All students must have one of these forms on file each year in order to use the school's computers.

- News Media Opt Out Form - Please complete this optional form if you do not want to authorize the print or electronic media or district officials to interview, quote, photograph or videotape your child or children during school events or activities during the school year, which are not open to the public.

- Notice of Refusal to Release Student Data - By not completing this optional form, the school may release certain information to colleges, the military branches, and others who may request the student information.

- Free or Reduced Lunch and Waiver Application - If you feel you may qualify for free or reduced lunches, please complete the packet. If accepted, the student's school fees will also be waived.

- Hale Electronic Communications Sign-Off Form - All students and their parent must sign off to this form indicating that they are aware of the policies and procedures regarding cell phones and other electronic items in the school.

- Change of Address/Phone/Household Information Form - If your address, phone, or any household information changes, then please fill out this form.

- Request for Tuition Waiver Due to Move Form - If you move out of West Allis or West Milwaukee and in order for your child to continue attending Nathan Hale High School, please fill out this form and bring it to the main office. You will need to meet with your child's assistant principal.

- Withdrawal or Transfer Form - Students withdrawing from our school must turn in this form on the student's last day of attendance. Be sure your child cleans out their locker of all personal items, returns their books, and pay all financial obligation.

- Athletic and Activities Code of Conduct and Agreement - All students participating in sports or other activities are held to the policies outlined in the code of conduct. All of these students and a parent must sign-off and agree to the code of conduct.

- Parent Directory & Volunteer Form - We ask all parents to complete this form and find a way to volunteer for one or several activities. Please turn it in by the first day of school each year.

- 2011-2012 Parent Teacher Student Association (PTSA) Membership Form - All parents should truly consider becoming a member of the PTSA. The PTSA is very active in supporting a number of programs in the school.

- 2011-2012 Booster Club Membership Form - If interested, please join the Athletics Booster Club by completing this form. The Booster Club supports all sports programs at Hale.
